Ocorrência do erro e processo de trabalho em enfermagem: imbricações no discurso

O erro é a consequência de
ações planejadas que não obtiveram o resultado desejado. Quando o erro é encarado
individualmente o processo de trabalho é deixado de lado (REASON, 2009). Logo,
estudar a relação entre o processo de trabalho em enfermagem e o erro contribui
para a prevenção, a detecção e a notificação do erro. Os objetivos deste estudo
são investigar
as relações entre erro e processo de trabalho em enfermagem no discurso
jurídico; analisar os efeitos de sentido de erro, em processos éticos julgados
pelo COREn/AL. Trata-se de uma pesquisa de campo (Parecer n.1374.867 –
CEP/UFBA), do tipo documental, de abordagem qualitativa, realizada a partir de
processos éticos de erro profissional, julgados em Conselhos Regionais de
Enfermagem do nordeste brasileiro, analisados à luz da Análise do Discurso
pecheuxtiana. Foram digitalizados 61 processos éticos, dos quais apenas 05
foram incluídos para análise, por atenderem aos critérios estabelecidos. Os
outros 56 processos estavam relacionados à negligência, abuso de
autoridade, imperícia, entre outros, sendo assim excluídos da pesquisa. A partir
da análise de processos éticos julgados pelo COREn/AL, percebe-se um
direcionamento do erro apenas à pessoa, excluindo a responsabilidade por parte da organização de saúde, como se esta não fosse responsável pelas
condições produtoras do erro. Os efeitos de sentido do erro podem trazer como consequência a
omissão do erro por parte dos profissionais sem resultar em aprendizado ou em
medidas futuras para a prevenção ou mitigação do erro. O COREn também acaba
silenciando o processo de trabalho no julgamento do processo ético, tornando
“culpado” apenas a trabalhadora em enfermagem. A análise dos efeitos de sentido de erro no
trabalho em enfermagem, em processos éticos julgados pelo COREn/AL,
possibilitou inferir que o caráter punitivo e individual causa um sentido de culpa que
pode ter como efeito a omissão do erro por parte da trabalhadora, prejudicando,
assim, numa possível prevenção de erro. O processo de trabalho está diretamente
relacionado ao erro. Portanto, é importante julgar o
erro através de uma abordagem sistêmica, o que contribui para a criação de
mecanismos de aprendizagem.
The mistake is a consequence of planned
actions that did not obtain the desired result. When the mistake is viewed
individually, the work process is left aside (REASON, 2009). Therefore,
studying the relationship between the nursing work process and the mistake
contributes to prevention, detection and notification of the mistake. The objectives
are to investigate the relations between mistake and nursing work process in
legal speech; to analyze the effects of the sense of mistake, in ethical
processes judged by COREn/AL. It is a field research (Legal opinion n. 1374.867
- CEP/UFBA), of documentary type, qualitative approach, carried out from
ethical processes of professional mistake, judged by Regional Nursing Councils
from the brazilian northeastern region, analyzed in light of the pecheuxtian
Speech Analysis. 61 ethical processes were digitized, from which only 05 were
included for analysis, for meeting the established criteria. The other 56
processes were related to negligence, authority abuse, malpractice, among
others, therefore being excluded from the research. From the analysis of
ethical processes judged by COREn/AL, it is perceived the mistake is directed
only towards the person, excluding the health facility’s responsibility, as if
they were not responsible for the conditions that caused the mistake. The
effects of the sense of mistake can bring as consequence the omission of the
mistake by the professionals without resulting in learning or future measures
of prevention or mitigation of the mistake. COREn also ends up silencing the
work process in the judgment of the ethical process, making only the nursing
worker “guilty”. The analysis of the effects of the sense of error in nursing
work, in ethical process judged by COREn/AL made it possible to infer that the
punitive and individual character causes a sense of guilt that can have as
effect the omission of the mistake by the worker, thus jeopardizing a possible
prevention of the mistake. The work process is directly related to the mistake.
Therefore, it is important to judge the mistake through a systemic approach,
which contributes to the creation of learning mechanisms.
