Situação em saúde da população indígena no Nordeste brasileiro

Os indicadores de Saúde são ferramentas utilizadas para entender a situação sanitária de uma determinada comunidade e a partir disso promover vigilância das condições de saúde. Diante disso, devido à importância de estimar a efetividade da gestão em saúde pública frente as minorias, este trabalho propõe traçar um panorama avaliativo das ações e planejamento de saúde pública dentro dos Distrito Sanitário Indígena (DSEI) distribuídos nos estados do Nordeste, no sentido de averiguar se há uma eficácia quanto às preocupações de acesso integral saúde indígena. O campo de estudo foi a área de abrangência dos Distritos Sanitários Especiais Indígenas (DSEI) da região Nordeste brasileira, cuja sedes estão localizadas em Alagoas, Bahia, Maranhão, Ceará, Pernambuco e Paraíba. DSEI do Estado de Sergipe fica localizado em Alagoas, logo, os dados foram analisados no mesmo DSEI. Os dados demográficos relativos à população indígena foram obtidos junto ao cadastro populacional registrado no Sistema de Informação da Atenção à Saúde Indígena (SIASI), no período de 2016 a 2020. Foram coletados dados demográficos: população total, razão de sexo, prorporção de menores de 5 anos, proporção de idosos, índice de envelhecimento e taxa bruta de natalidade . Em relação aos dados de saúde pública, foram obtidos dados sobre: esquema vacinal completo, taxa de mortalidade, incidência de doenças infecto-contagiosas e não contagiosas, incidência de neoplasias malignas, Número de atendimentos médicos (SUS) por habitante, cobertura de consultas de pré-natal e classificação do estado nutricional de crianças indígenas menores de 05 anos. Foram detectados 31 etnias, distribuías em 386 aldeias, 59 municípios, com 64.886 indíviduos. Os estados com maior contingente populacional foram PE e MA, com 19.730 e 16.179 habitantes, respectivamente. Em relação ao número de homens para cada 100 mulheres, há uma predominância da população masculina em torno de 51%, em relação à feminina, no geral. Quando observada a taxa de natalidade, houve uma proporção de maior número de jovens em relação ao de idosos. Em relação à proporção de atendimento médico, observou-se que, de 2017 a 2022, a taxa variou de um estado para o outro, sendo 0,856 para AL e SE, 0,792 para BA 0,906 no CE, 0,134 no MA, 0,928 em PE e 0,832 para a PB. No que tange às doenças não transmissíveis, os dados de incidência foram relativamente baixos para cálculo da taxa. Em dados gerais, PE teve uma incidência maior de 0,52%, seguidos de PB e CE, BA, e Al e SE, com 0,34%, 0,32%, 0,19%, 0,16% respectivamente. O Estado do MA não registrou doenças transmissíveis analisadas nesse estudo. Em relação às doenças transmissíveis, observa-se que a Sífilis foi a IST mais prevalente para todos os estados, exceto o MA. A prevalência dessa patologia variou de 0,11% no DSEI de Al e SE a 0,47%, para o DSEI de PE. Foram registrado 744 óbitos por Doenças Crônicas Não Transmissível (DCNT). Observou-se que as doenças cardiovasculares tiveram maior taxa de letalidade dentre as demais DCNT, totalizando 495 mortes, onde os estados de PE e MA apresentaram 148 e 149, respectivamente. Já as patologias do aparelho genitourinário e diabetes tiveram as menores proporções de óbitos. Sobre a cobertura vacinal, notou-se que, 56,7%, do total de crianças menores
de 01 ano atendidas não atingiu a meta estabelecida, exceto para o ano de 2017,
cuja proporção foi de 91,1%. Quanto ao esquema vacinal das crianças indígenas
de 01 a 04 anos, a meta foi alcançada durante os 05 anos, tendo o ano de 2018
com maior percentual obtido, com 94,7% de imunizados e tendo o menor índice
alcançado o ano de 2016, com 90,4%. O Estado de PE teve o maior percentual de
vacinação (95,56%) e o MA teve o menor (86,54%). Em relação às condições de
pré-natal, no ano de 2016 houve uma maior desassistência a este tipo de procedimento,
onde, em média, 30% das grávidas não chegaram a realizar nenhuma consulta, com
destaque para os DSEI de PE e PB que não contabilizaram seus dados em nenhum
dos levantamentos. O DSEI do MA foi o estado que observou maior taxa de
desassistência, totalizando 46,7. No que tange à porcentagem de crianças com muito baixo peso, os dados
ficaram entre 0,2% e 2,7%, o que denota uma característica da eficiência das
ações de combate à desnutrição.
Health indicators are tools used to
understand the health situation of a given community and, based on this,
promote surveillance of health conditions. In view of this, due to the
importance of estimating the effectiveness of public health management
vis-à-vis minorities, this work proposes to outline an evaluative overview of
public health actions and planning within the Indigenous Sanitary District
(DSEI) distributed in the Northeastern states, in the sense of to verify if
there is an effectiveness regarding the concerns of integral access to
indigenous health. The field of study was the area covered by the Special
Indigenous Health Districts (DSEI) of the Brazilian Northeast region, whose headquarters
are located in Alagoas, Bahia, Maranhão, Ceará, Pernambuco and Paraíba. DSEI of
the State of Sergipe is located in Algoas, therefore, the data were analyzed in
the same DSEI. Demographic data related to the indigenous population were
obtained from the population registry registered in the Indigenous Health Care
Information System (SIASI), from 2016 to 2020. Demographic data were collected:
total population, sex ratio, proportion of children under 5 years, proportion
of elderly, aging index and crude birth rate . Regarding public health data,
data were obtained on: complete vaccination schedule, mortality rate, incidence
of infectious and non-contagious diseases, incidence of malignant neoplasms,
number of medical consultations (SUS) per inhabitant, coverage of prenatal care
and classification of the nutritional status of indigenous children under 05
years of age. 31 ethnic groups were detected, distributed in 386 villages, 59
municipalities, with 64,886 individuals. The states with the largest populations
were PE and MA, with 19,730 and 16,179 inhabitants, respectively. Regarding the
number of men for every 100 women, there is a predominance of the male
population around 51%, in relation to the female population, in general. When
observing the birth rate, there was a proportion of a greater number of young
people in relation to the number of elderly people. Regarding the proportion of
medical care, it was observed that, from 2017 to 2022, the rate varied from one
state to another, being 0.856 for AL and SE, 0.792 for BA, 0.906 for CE, 0.134
for MA, 0.928 for PE and 0.832 for PB. With regard to noncommunicable diseases,
the incidence data were relatively low for calculating the rate. In general
data, PE had a higher incidence of 0.52%, followed by PB and CE, BA, and Al and
SE, with 0.34%, 0.32%, 0.19%, 0.16% respectively. The State of MA did not
record communicable diseases analyzed in this study. Regarding transmissible
diseases, it is observed that Syphilis was the most prevalent STI for all
states, except MA. The prevalence of this pathology ranged from 0.11% in the Al
and SE DSEI to 0.47% for the PE DSEI. There were 744 deaths due to Chronic
Noncommunicable Diseases (CNCD). It was observed that cardiovascular diseases
had a higher lethality rate among the other NCDs, totaling 495 deaths, where
the states of PE and MA had 148 and 149, respectively. On the other hand,
pathologies of the genitourinary system and diabetes had the lowest proportions
of deaths. Regarding vaccination coverage, it was noted that 56.7% of the total
number of children under 01 years of age attended did not reach the established
target, except for the year 2017, whose proportion was 91.1%. As for the
vaccination schedule for indigenous children aged 01 to 04 years, the goal was
achieved during the 05 years, with the year 2018 having the highest percentage
obtained, with 94.7% of immunized individuals and having the lowest rate
reached in the year 2016, with 90.4%. The State of PE had the highest
percentage of vaccination (95.56%) and MA had the lowest (86.54%). Regarding
prenatal conditions, in 2016 there was a greater lack of assistance for this
type of procedure, where, on average, 30% of pregnant women did not have any
consultations, with emphasis on the DSEI of PE and BP that did not accounted
for their data in any of the surveys. The DSEI of MA was the state that
observed the highest rate of non-assistance, totaling 46.7. With regard to the
percentage of children with very low weight, the data were between 0.2% and
2.7%, which denotes a characteristic of the efficiency of actions to combat
malnutrition.
